Cell Phone Request Form (Must have prior approval of Vice Chancellor of Administration and Finance. The policy)
A Wireless Communications Device Issuance Request form is required before ordering cellular and/or data services. The form must be completed with an explanation of use and signatures of approval. Return completed form and detailed order request to telcom@olemiss.edu or fax to 662-915-7010. The purchase of a wireless device must be awarded to one of the two state vendors, using the lowest quote for the device or service.
